Rates and Insurance

Insurance
I currently an in-network provider for the following insurers:
   Health Choice
   Blue Cross Blue Shield (BCBS)
   United Health / Optum Behavioral Health
  
If you do not have coverage on any of these plans, we can arrange a cash rate and you may self-file for reimbursement from your insurance company.  If you plan to utilize your insurance, please contact your provider to verify how your plan will cover behavioral health services.
I recommend asking the following questions to your insurance provider to help clarify your benefits:
  - Does my health insurance plan include mental or behavioral health benefits?
  - Do I have a deductible? If so, what is it and have I met it yet?
  - Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?
  - Do I need written approval from my primary care physician in order for services to be covered?

Private Pay
If you are uninsured or prefer not to involve insurance, hourly rates are available.

Methods of Payment
Payment is due at the time of service. Cash or check are preferred. Credit cards are also accepted. There is a $35 fee for all returned checks.

Cancellation Policy
24 hours notice is required to change or cancel an appointment. Failure to do so will result in a fee.
